Every patient must bring in valid California identification as proof of your California residency. You must be a California resident to be eligible under California state medical marijuana laws. If you do not have a California license or I.D. card, an out of state I.D., passport, or other form of photo I.D. is acceptable with proof of California residency. This could be a bank statement, utility bill, etc.
You must also bring the appropriate fee for your visit. At this time, our office is not accepting checks or credit cards. If payment is an issue, please speak with one of our office staff.
New Patients: Please bring in any documentation you have from your primary care physician that states your diagnosis.
This may include:
- Doctor reports/summary of diagnosis, findings
- Treatment notes
- A letter specifically to our office
...or anything else from your doctor that tells us what your diagnosis is.
If you do
not have access to your medical records, we can assist you in obtaining them. If
you do not have a doctor, are uninsured, or have not had a check up in a while,
please click here for a list of clinics and counseling options.
Please bring any medications you have been prescribed, whether or not you are currently taking it.
Renewing Patients: Please bring any updated paperwork you have regarding your condition.
